We have assistance programs you may qualify for:
Huntsville Residents - Fixin' AL and Madison County Residents - SNAP program
***SNAP FUNDING IS OUT UNTIL NEW FISCAL YEAR-CHECK IN MID OCTOBER FOR NEW FUNDS***
To qualify for these programs, you must provide documentation at time of appointment.
WIC or EBT card
Social Security Disability Benefit Letter
Drivers License with current address or utility bill in same name
**BE SURE YOU NOTE IN THE APPOINTMENT NOTES THAT YOU ARE REQUESTING AN ASSISTANCE PROGRAM**
Outside of Madison County - please call our office Mon-Thurs. 9am-3pm for options.
A $20 deposit is required for each appointment when you schedule.
This deposit will apply toward your surgery cost.
If your program cost is $5, we will refund your $15 on the day of
the appointment or apply it to requested services not covered under program.
If you do not show for a scheduled appointment you will forfeit your full deposit and will not be eligible for the program in the future.
A 24 hour cancellation notice is required by calling 256-489-0418 or email firstname.lastname@example.org
If you cancel your appointment you will have 2 weeks to reschedule or your depositbecomes a donation to our clinic.
